Xverity Launches [email protected] Hub
First comprehensive on-demand resource service
for patients and caregivers to avoid hospital readmissions
Bethlehem, PA January 17, 2018 – Xverity, Inc., today announced the launch of its first-of-a-kind [email protected] Hub created for health care providers to better serve the social and health-related needs of patients discharged from the hospital or other health care institution and their care givers. The Hub provides users with on-demand access to transportation, medication therapy management, home delivery of prepared meals and groceries, curated education videos for patients and caregivers and concierge services such as housekeeping, bookkeeping, event planning, errands and other household tasks. To support caregivers, the Hub includes a special caregivers feature that provides information and enables caregivers to participate in peer group exchanges.
Xverity founder and CEO, Joe Cellucci, said the impetus for the [email protected] Hub was his own experience as a Lymphoma cancer survivor and as primary caregiver for his mother. “After being discharged from the hospital, I felt I was totally on my own. Same with my mother when she left the hospital. It was often frustrating trying to get the information and services needed to implement what we call a 360 Life Plan. As a serial entrepreneur developing technology solutions to problems, I knew that the break in the continuum of care could be repaired and improved. The [email protected] Hub is a high touch/high tech solution made possible through our proven technology platform and partnerships with health care leaders like Qualcomm, PharmPro, RoundTrip, Milner-Fenwick and others.”
Cellucci said the company is signing agreements with national, regional and local brands connected to the Hub as preferred service provides for each service category, including transportation, medication adherence, home-delivered prepared meals and groceries, education, home maintenance and repairs and concierge services.
Users can access the Hub in multiple ways, ranging from in-home Amazon Connect voice commands, to telephone requests direct to the designated service provider or to Xverity’s Advocacy Service Center, a dedicated contact center with trained staff to answer questions, expedite requests or troubleshoot problems.
“We’re also introducing innovations to make using the [email protected] Hub convenient and rewarding. For example, a special payment card we have negotiated for the Hub can be used to pay for most services accessible through the Hub. And we are signing with a leading health incentive provider to encourage compliance with the 360 Life Plan; for example, medication adherence and following a personalized meal plan,” Cellucci said.
Xverity’s innovation officer, Linda Whitman, PhD, said that the [email protected] Hub can be configured for the needs of users, especially high-risk patients, under the care of PACE programs, Accountable Care Organizations, home care and home health care agencies and health care systems. She said that because the [email protected] Hub can improve the efficient delivery of clinical care to reduce costs and avoid hospital readmissions, payers will also benefit.
