An inside look at Xverity’s Patient@Home Hub
When Joe Cellucci was caring for his ailing father following discharge from the hospital, the cracks in the healthcare system that aggravated Joe and his family inspired him to create a solution to a problem that challenges millions of patients and their caregivers. The divide between inpatient care and the transition to home care was not new to Joe. A cancer survivor, he had personally endured the disconnects in health care.
A successful serial IT entrepreneur, Joe’s is another story of an innovator who saw a problem that touched him and his family, stepped back and said, “There has got to be a better way!” Joe subsequently became a student of care management outside of the hospital setting and in 2015 launched what is today Xverity. Joe’s solution combines innovative healthcare IT with best practices from the hospitality industry to provide on-demand, single-point access to clinical and social services. This year, the firm introduced the first-of-a-kind Patient@Home Hub to provide patients and their caregivers easy, reliable managed access to preferred providers who help ensure the safety, security and convenience of patients whose care is being managed @Home, just like Joe’s dad.
In a hospital setting, patient services are connected and coordinated under the auspices of one or more medical staff. Once a patient is discharged, needed services are disconnected and not easily accessible to the patient and his or her caregivers. Research shows that the lack of connectivity and coordination can have an adverse effect on the patient, creating emotional stress and anxiety resulting in the need for emergency care and hospital readmissions. The problem is further complicated by the pressure of rapidly changing economics on the healthcare industry as it transitions from a “Fee for Service” model to a “Pay for Performance” model.
Designed with input from clinicians and home care professionals, Xverity’s three-dimensional product – Evaluation Suite, Activation Suite and Orchestration Suite – is configured to ensure that the patient and his/her caregiver can participate in accessing and managing the delivery of myriad medical services. The @Home services menu can include Home Health, Physical Therapy, Medication and Infusion, as well as social services, such as Transportation, Meals, Groceries, Education, Home Repair and Maintenance and a bundle of concierge services.
Xverity’s Patient@Home Hub and Advocacy Services are activated before the patient is discharged from the hospital, and are in place, anticipating the patient’s needs and preferences, before the patient reaches his/her front door. Just as the best hotels use technology to “know” their guests intimately before they arrive in the lobby and assign someone to the guest to ensure personalized service, Xverity’s Patient@Home Hub is complemented by a trained Advocate to ensure that the personalized Patient@Home care plan is clearly articulated and implemented with compassion and professionalism.
The need for the Patient@Home Hub was recently underscored by Dr. Kevin Volpp, director for the Center for Health Incentives and Behavioral Economic and vice chairman for health policy at the University of Pennsylvania. In a recent blog, Dr. Volpp wrote, “physicians are rarely trained to help patients change their behaviors, but a tech-based system could help solve the ‘5,000 hours problem’ that plagues the industry – the time that patients spend on their own, away from the watchful eye of a provider.”
The greatest need for Xverity’s solution is among PACE Programs, ACOs, Health Systems and payors. Depending upon the need and circumstances, Xverity can configure its product suites to address specific issues – such as assessment and risk stratification, patient engagement using mobile devices and other communication tools, service delivery and monitoring and field care personnel management.
